Vaccines and Vaccine Preventable Diseases

My research efforts are focused on improving our understanding of the cell mediated immune response to vaccines. Currently, vaccines are licensed based on their ability to induce an antibody response and prevent infection. This approach largely ignores the contribution of T cells to protective immunity.

My laboratory is studying cytokine responses as markers of T cell activation following immunization. We found interesting positive correlations between psychosocial factors and cytokine production following influenza and hepatitis A immunization in a group of aging adults. Several examples of psychological stress negatively affecting vaccine response have been found, but ours may be the first report of high well being predicting good vaccine response. We are continuing this work in a cohort of elderly women. This work contributes to investigations of the biology behind lower immune responses following immunization in older individuals. We hope to develop strategies to improve the vaccine response in the elderly-either with better vaccines or alternative vaccination strategies.

In addition to these studies, we are also pursuing our interest in improving the health of individuals in Wisconsin. We completed a pilot study to measure tetanus antibody concentrations in Wisconsin farmers. Surprisingly, we found that the Wisconsin farmers had higher tetanus antibody concentrations than nonfarmers in our study. Consistent with the findings of other investigators, we found that older women were much more likely to be unprotected from tetanus than younger individuals.

Our laboratory is also interested in strategies to vaccinate patients who have received solid organ transplants. This patient population is a particularly high risk. They receive immunosuppressive drugs to prevent organ rejection, but this lowers their ability to fend off infection and their ability to mount vigorous responses to immunization.

As a pharmacist, I maintain a clinical interest in the section of the population that is the highest user of medications-the elderly. This population is at higher risk of adverse events associated with medication use because of biology and the use of multiple medications simultaneously.
